Waiver:
I hereby give permission for my child to participate in Sky Blue Soccer School programs. I assume all risks in regard to the programs in which my child will participate. I release, indemnify and agree to hold harmless Sky Blue Soccer School, it's directors, officers, coaches, trainers and volunteers from any liability that may arise from participation
in the programs organized by Sky Blue Soccer School. By agreeing to these terms, I
attest to the following: that the information entered is correct, and in the
event of a medical emergency, I authorize the Sky Blue Soccer School staff to seek emergency
medical care for my child as deemed necessary.
